STATE REQUIREMENTS

Age Requirement The state of Nebraska requires that all children beginning kindergarten be five years of age before or no later than October 15.

Birth Certificate It is required to have a copy of each child's birth certificate on file in his/her permanent student file.

Physical Examination:  Kindergarten and 7th grade students are required to have a physical examination prior to starting school.  (An examination given up to six months prior to entrance into school is acceptable.)  A dental examination is also recommended at this time.  Out of state students in grades 1-8 are required to have a physical examination prior to starting school.

Immunizations The Nebraska school law requires that all students be immunized against diphtheria, pertussis, tetanus, poliomyelitis, hepatitis B, measles, rubella (German or 3-day measles) and mumps.  Immunizations will not be given at the school.  We require a certified copy of each student's shot record.  This is returned to you after a copy is made for his/her permanent student file.  See your physician or call Douglas County Health Dept., 444-6163 or Tri-County (Sarpy) office, 334-1820, for more information about clinics and physical exams.
