STATE REQUIREMENTS
Age Requirement: The state of Nebraska requires that all children beginning kindergarten be five years of age before or no later than October 15.
Birth Certificate: It is required to have a copy of each child's birth certificate on file in his/her permanent student file.
Physical Examination: Kindergarten and 7th grade students are required to have a physical examination prior to starting school. (An examination given up to six months prior to entrance into school is acceptable.) A dental examination is also recommended at this time. Out of state students in grades 1-8 are required to have a physical examination prior to starting school.
Immunizations: The Nebraska school law requires that all students be immunized against diphtheria, pertussis, tetanus, poliomyelitis, hepatitis B, measles, rubella (German or 3-day measles) and mumps. Immunizations will not be given at the school. We require a certified copy of each student's shot record. This is returned to you after a copy is made for his/her permanent student file. See your physician or call Douglas County Health Dept., 444-6163 or Tri-County (Sarpy) office, 334-1820, for more information about clinics and physical exams.
